Teaching and Non-Teaching Adjuncts Only – Spring 2014 Benefits Enrollment Notification

Please be reminded that the PSC-CUNY Welfare Fund deadline to submit new Adjunct Benefit Enrollment Applications is April 15th, 2014. The coverage effective date will depend on the enrollment request submission date. Please note the following criteria outlined below must be met in order to be eligible for Adjunct health coverage through the PSC-CUNY Welfare Fund:

You are not covered by or eligible to be covered by other basic health insurance by virtue of employment of self or spouse or through government entitlement.

You have fulfilled the continuity requirement either as a:

Teaching Adjunct: Two consecutive semesters of adjunct instruction at CUNY immediately prior to the current semester, or

Non-Teaching Adjunct: Two consecutive semesters of at least 15 hours per week at CUNY immediately prior to the current semester. Please note PSC-CUNY recently changed this requirement from 10-15 hours per week; however, will allow 15 hours per week for the prior Fall 2013 semester and 10 hours per week for the Spring 2013 semester, to count toward eligibility. Employees must be working a minimum of 15 hours per week during the current Spring 2014 semester.

You fulfill the current hours requirement either as a:

Teaching Adjunct: Carries a current teaching load of six or more hours per week at one or more than one CUNY institution (combined), or

Non-Teaching Adjunct: Is currently working at least 15 hours per week at CUNY.

Health Insurance Options:

Your coverage options are the Empire Blue Cross HMO and HIP Prime HMO Plans. Both provide in-network coverage ONLY and require the selection of a Primary Care Physician. Minimum optical and dental coverage is provided through the HIP Prime Plan only. Members can also access a vision pass through General Vision Services by calling 800-884-3332 and referring to account number 4009. Please refer to the Adjunct Optical Plan document for further information on this benefit.

If you meet the eligibility requirements and wish to enroll, please complete the 2014 Adjunct Health Insurance Enrollment Form. If electing the Empire Blue Cross HMO plan, please complete the Adjunct Empire Blue Cross Enrollment Form. If electing the HIP Prime HMO plan, please complete the Adjunct HIP Enrollment Form. Please be advised that we have been informed by the PSC-CUNY Welfare Fund that effective 02/01/2014, the HIP Prime HMO Plan rates for a family coverage have changed. Please refer to the 2014 Adjunct Health Insurance Enrollment document for further information on these new rates.

All completed forms must be returned to the Office of Human Resources between February 1st and April 15th. Please refer to the Summary Plan Description and the Empire HMO and HIP Prime HMO Benefit Summaries for additional information.

Please note, if you are currently enrolled and continue to meet the eligibility requirements as outlined in the Summary Plan Description, you do NOT need to do anything to maintain your current coverage.
