Required: Photo ID
Required: Social Security cards of yourself, your spouse, and any dependents
a copy of last year's return if possible for the preparer's reference
W-2, 1099-MISC, 1099-INT and all other forms that you, your spouse, or your dependents received for 2010
Notice 1378 or Economic Stimulus Payment amount if you received the Economic Stimulus Payment
Real Estate Tax Receipts if you paid real estate taxes
Schedule of Business Expenses if you received 1099-MISC and would like to deduct some expenses
Traditional IRA contribution amount if you have a traditional IRA account
Daycare center name, address, & tax ID if you had child care expenses
Bank account number & routing number if you would like to get your refund deposited directly into your account
All other forms related to tax
