GUIDELINES FOR OBTAINING A SEVIS I-20

In order to issue the SEVIS I-20 we need the following supportive documentation:

1. CONFIDENTIAL DECLARATION AND CERTIFICATE OF FINANCE FORM (CDF)

Along with this completed form please provide us with the following original documents, translated to English, not older than 3 months:

Note: All documents must be:

  • Originals (unless stated otherwise faxes and copies will not be accepted)
  • Translated in English
  • Within the last 3 months (documents older than 3 months will not be accepted)

If you are supporting yourself

  • BANK STATEMENT in your name, which states the date the account was opened, current balance, average deposits and average balances. The bank statement must be an official bank statement on letterhead stationery.
  • PROOF OF INCOME :

    Any of the following:

    • Property estimate issued by the certified accountant
    • Certificate of bonds or stocks (copies and faxes are acceptable)

If you cannot provide any of the above Proof of income documentation, you have to show funds on your bank statement for all years you will be attending Baruch College.

If you are receiving cash support from an individual or company

  • AFFIDAVIT OF FINANCIAL SUPPORT completed by each individual sponsor. This is a letter where your sponsor states how much money and the amount of time he/she will continue to sponsor you.
  • BANK STATEMENT in the name of the sponsor, which states the date the account was opened, current balance, average deposits and average balances. The bank statement must be an official bank statement on letterhead stationery. If the account is in the name of more than one person, each individual must submit a separate affidavit of support.
  • PROOF OF INCOME:

    If the sponsor is an individual: any of the following:

    • Verification of employment on letterhead stationary. This document has to include salary (per month or per year) and the currency. If self-employed- income estimate prepared by a bank or private accountant
    • The most recent income tax documentation (for US citizens W2) (copies and faxes are acceptable)
    • Property estimate issued by a certified accountant
    • Certificate of bonds or stocks (copies and faxes are acceptable)

    If the sponsor is a Company:

    • The most recent “profit and loss statement” for the company. (copies and faxes are acceptable)

     

If you are receiving free room and board

  • AFFIDAVIT OF FREE ROOM AND BOARD
  • PHOTOCOPY OF LEASE, RENT RECEIPTS OR DEED

 

2.COPY OF YOUR VALID PASSPORT (IDENTIFICATION PAGE)
