We in Student Disability Services (SDS) plan to streamline exam proctoring in Spring 17 to better serve students and faculty. To that end, we want you to know in advance about the change to exam proctoring procedures which we are implementing.
It is the student’s responsibility to identify themselves to you at the beginning of the semester as a student with a disability. A student with a disability who requests to take exams outside the classroom will now have an Exam Proctor Form and will ask you to complete a short section of this Form. (A copy of the form is attached to this email for your perusal.) It is the student’s responsibility to return the completed Exam Proctor Form to SDS. When we receive the completed form from the student, you will receive an email in a timely fashion from firstname.lastname@example.org asking you to confirm the exam details and requesting that you reply to the email with the exam attached.
Administering exams is a responsibility which we take very seriously. Students who take exams in our lab are proctored. The lab is under video surveillance at all times, and students’ phones and backpacks are placed in lockers. Introducing the proctor form will make the entire process more secure, hold students more accountable, and most importantly maintain the highest level of academic integrity.
