I request that an authorization code be assigned to me so that I can make long distance telephone calls from Baruch College while pursuing College business. I understand, and agree to abide by New York State policy that precludes personal telephone calls from being paid for by Baruch College. Should I need to make a personal long distance call from Baruch facilities, I agree to reimburse Baruch College for the cost of the telephone call(s) that I make.

I understand that long distance services may be terminated as a result of non-payment of long distance bills or misuse of my assigned code.

Note: Authorization codes are assigned to full-time faculty and staff only. Exceptions must be explained in a letter from the Chair and sent to voicecom@baruch.cuny.edu.

You may pick up your authorization code from BCTC (151 E. 25 th St. – 9 th floor). Please bring your college id.
